By participating in programs and activities at Mosaic Quarter (MQ), I acknowledge and agree that:

1. ASSUMPTION OF RISK: I understand that participation in sports and recreational activities carries inherent risks, including but not limited to physical injury, equipment damage, and illness. I voluntarily assume all such risks.

2. RELEASE OF LIABILITY: I, on behalf of myself and my household members, hereby release Mosaic Quarter, its owners, operators, employees, coaches, and volunteers from any and all liability, claims, demands, or causes of action arising from participation in MQ programs or use of MQ facilities.

3. MEDICAL AUTHORIZATION: In the event of an emergency, I authorize MQ staff to obtain emergency medical treatment for myself or my minor children if I cannot be reached.

4. MEDIA CONSENT: I grant Mosaic Quarter the right to use photographs or video recordings of myself and my household members for promotional or marketing purposes.

5. CODE OF CONDUCT: I agree to abide by MQ's Code of Conduct and understand that violations may result in removal from programs without refund.

This waiver is binding on me and my legal heirs, assigns, and representatives.
